PLEASE NOTE:  Please make a copy of the PHYSICAL and place the copy in with the completed athletic packet to turn in to LMS when trying out for a sport.  Keep your origianal PHYSICAL in the event that your son or daughter will be trying out for a sport later on in the schoolyear.  Thank you.
