Americans with Disabilities Act (ADA) - Requesting Reasonable Accommodation
The Office of Equity and Inclusion/Professional Standards (OEI/PS) reviews requests for reasonable accommodation under the ADA (Americans with Disabilities Act Amendments Act, as amended).
To request accommodation needed because of a disability, the employee should complete and return our ADA Accommodation Request Form. Our office will follow up with the employee if additional documentation is needed.
After the documents are reviewed, a meeting will be held with the employee, their supervisor, and OEI/PS. The requested accommodation will be discussed to determine whether the accommodation is reasonable, and any alternatives that may be needed.
ADA accommodation is provided for the current school year, unless a medical or administrative change necessitates an adjustment to the accommodation. OEI/PS will contact the employee at the beginning of each school year to assess whether the current accommodation is still effective or changes need to be made.
Please read our ADA Accommodation Request Procedures or contact our office at 904-390-2181 for additional information.
