• In the event of a serious injury, call 911 immediately and then call school police. 

    The school or worksite Workers’ Compensation Designee should be notified as soon as possible thereafter.  For non-emergency injuries, contact the school or worksite’s designated Workers’ Compensation Designee as soon as basic facts are known; the Workers’ Compensation Designee will refer the employee for medical care. The Workers’ Compensation Designee will also complete the online Workers’ Compensation First Report of Injury (FROI) for employee injuries or an online Student/Visitor Accident Report (SVAR) for other injuries.  If you are unsure who your school’s Workers’Compensation Designee is, contact the school’s main office, or contact Grace Noonan in the Risk Management Office at (904) 390-2384 or NoonanG@duvalschools.org.

    Supervisors (the person who the injured worker reports to) of employees who are injured at work should conduct accident investigations on behalf of Risk Management.  The supervisor should complete the Accident Investigation Report and submit it to Risk Management at Safety@duvalschools.org within 48 hours of the accident. These reports may not be shared with others, except with the site administrator, for purposes of preventing future accidents of a similar type.  This investigation is distinct from and in addition to any Workers Compensation or Liability investigation that may be conducted by a Johns Eastern insurance claims adjuster.
