Diabetes Management Program 

С³óߣ offers diabetes management for those already diagnosed. The program consists of ongoing education classes, health coaching, monitored lab work, and annual foot & eye exams. The program is offered at no cost to you, and by participating, you will receive FREE medication, supplies, and support.  Call Jennie Cruz at (904) 390-2323 for more information and how to register. 

Eligibility: All employees, retirees, & dependents (18+) enrolled in С³óߣ’ health plan who have a confirmed diagnosis of diabetes

Diabetes Prevention Program

Florida Blue and С³óߣ have teamed up with Solera to bring you a health and wellness program built with you in mind. When you sign up, you’ll get the tools you need to help you make healthy food choices, move more, and lower your risk of type 2 diabetes.

You’ll have access to leading wellness programs such as WW (Weight Watchers® Reimagined), Betr Health, and local community programs. The program is available at no additional charge to Florida Blue members who qualify.
